Supplementary Information Forms (SIF) for secondary schools

Some schools ask for a Supplementary Information Form (SIF) or Certificate of Catholic Practice (CCP) in addition to their application form.

These are:

You must send the completed form directly to the school. If you do not complete and return the SIF or CCP, your child may not be offered a place at the school.
