Working Together to Safeguard Children 2023 sets out requirements for “child death review partners” (consisting of local authorities and any clinical commissioning groups for the local area). This is to make arrangements to review child deaths through a specific process.
The child death review reflects in detail on the history of the involvement of the different agencies across all sectors with every child death. It aims to consider any lessons to be learned, issues to be addressed, or opportunities for improving services. Below are some useful links:
