Who we are and what we do
The Short Break service provides a chance for disabled children to spend time away from their carers. They'll have the chance to try out new things, have fun, and make new friends. Short breaks can give families a chance to have a break from their caring duties and to do things together.

What information we hold about you
In order to provide children and young people with a short break or a domiciliary support service, we collect the following information from:
- Name
- Date of birth /Age
- Full Address and Postcode
- Telephone/Mobile contact numbers (parents/carers)
- Email Address of parent/carer
- Claiming DLA/PIP (Yes/No answers)
- Disability /Additional Needs
- Communication Needs
- Mobility/Personal Care
- Sensory Needs/Awareness of danger, Therapies, medication, Specialist equipment, accessing activities outside of the home
- Sleep issues
- Education information (e.g. EHCP)
- Mosaic number/Capita Number
- Diagnosis of disability
- Parents/Carers personal information (Full Name)
- Sibling (Full name if under 18 and age) who live in the same household
- Details of siblings if under age 18 who live in same household
- Details of siblings disability if under 18 who live in same household
- Ethnicity
Why we need your information and how we use it
The processing and use of your personal data are underpinned by our compliance with our legal and statutory obligations. This is primarily under section 25 of the Children & Young Persons Act 2008 and the Breaks for Carers of Disabled Children Regulations 2011 which requires local authorities to provide short Breaks services that will help and support parents and carers who look after a disabled child or children.
We consider your privacy important and we will only use your personal information to provide the Short Breaks or domiciliary service to you. We will also use it to track the level of support that each child/young person uses within a financial year.
The lawful basis for the processing
The lawful basis for processing your data is consent and public task.
In most cases, we will obtain your personal data directly from you. This will be obtained from you by completing a self-referral form.

We will only share your data, or the data of your child, where you have provided consent by completing a referral form.

The information you provide will remain confidential. The information will only be shared with commissioned Short Break and domiciliary care providers.

How long we will keep your information
