What can children and families expect from us?
Children and families can expect the following:
- An allocated worker for the young person and the family
- The worker will visit the child at home, school or wherever they are comfortable with, to get to know them better and what help/support they might need
- The worker will also look at all the ways that the parents might be helped and what their needs are in order to better support them
- Everything discussed with the worker will be confidential, and will not be shared with another agency unless consent is given
- The worker will carry out an assessment of the young person and families needs
- The worker will then put together a support plan after discussing it with the young person and the family
- The young person and the parent will agree some commitments they will make towards the plan in return for the support
- The Plan (ISP) will be taken to a multi agency panel that meets every month to be agreed
- The worker will then try and implement all the recommendations made on the plan that will hopefully support the young person
- The Plan will then be reviewed half way through the YISP involvement to see if it is working or whether it needs to be amended to make it work better
The young person and family will be involved in discussing with the worker what their Exit Plan should look like and what long term support they need. The YISP will then try and find out who can provide this long term support via the Panel. The Exit Plan is there to make sure that the family are not left isolated after receiving intensive support over 6 months, but are confident in accessing support whether that's independent of any agency or in partnership with mainstream service providers.
