- Self referral by contacting the programme administrator on 0208 430 7426 or by email to nasima.begum@walthamforest.gov.uk
- Referral by your GP, nurse or other health professional - ask your GP or practice nurse to complete the appropriate referral form or contact our administrator directly on 0208 430 7426
