The Household Health Improvement Service has been set up to help residents in our priority areas to live a healthier lifestyle, especially people who have been identified as being most at risk of developing coronary heart disease and cancer. To help us achieve this we have a team of five health professionals called Health Improvement Managers working in all the Better Neighbourhoods areas.
Health Improvement Managers offer 1-to-1 support to patients wanting to improve their lifestyles and improve their well-being. Household Health Managers are able to see patients either at the patient's home, or at a community base such as a health centre or community centre. They identify client’s needs using a Health Needs Assessment. They also provide advice on a number of ways to improve health including:
- physical activity
- stop smoking support
- weight management
- sensible drinking
- healthy eating
Health Improvement Managers also help you access other relevant local services.
