How to use these online forms

The links to the forms are on the right. The form will open in a new window. Just fill it in and submit it. You only have to fill in the pages that apply to you and you can save the form part-way through if you want.

Print out and sign the declaration and any other relevant forms, then

  • take them with your evidence to the Customer Service Centre or Leyton Library
  • or post them to:

If you don't have access to a printer, there is space on the form to ask us to print a copy of the declaration and send it to you.

We will contact you to make an appointment to come in with your evidence and sign your declaration. Please bring in original documents as we can't accept copies as evidence.
