Transition to adulthood information packs
These packs aim to provide a variety of information and contact details of services an support available to young disabled people and their parents/carers.
- Transition to Adulthood Easy Read Pack (1.9MB PDF file) is aimed at young disabled people. It provides a wealth of information in plain English with graphics
- Transition to Adulthood Information Pack (1.7MB PDF file) which holds in depth information regarding the various aspects of transition to adulthood for parents, carers and professional alike
