The Children's Community Nursing Team (CCNT) work with children, families, carers, nursery and school staff to enable children to attain and maintain their optimum level of health within their home and school environment. CCNT may also provide supplies of medical equipment when necessary.

CCNT support and enable parents/carers to care for their sick or injured child at home, reducing the trauma caused by hospital admissions and attendances, and ensuring that there is as little disruption as possible to the routines of children and their families.

Who are we?

We are a team of eight paediatric community nurses.  Each member of the team is responsible for a generic, geographically-based caseload, encompassing Waltham Forest.

What do we do?

We provide skilled, high quality nursing care, advice and support for families with children aged 0-16 years who have acute or chronic conditions, including -

  • diabetes
  • respiratory disorders - asthma, cystic fibrosis, bronchiolitis
  • oncology and haematology (usually shared care with a Tertiary Centre)
  • eczema
  • wounds, scalds and burns
  • HIV/AIDS
  • any other condition requiring paediatric nursing input

Or who require the following treatments -

  • neonatal care - home oxygen, tube feeding
  • administration of intravenous medications
  • infusions of blood if appropriate
  • enteral feeding
  • co-ordination of packages of care for children with complex nursing needs
  • post-operative checks
  • palliative care

What area do we cover?

We see children living in the Leytonstone, Leyton, Chingford, and Walthamstow areas (within Waltham Forest).

How does the service operate?

Our service presently operates between 8.30am to 4.30pm, 6 days a week Monday to Saturday, and it is our intention to extend our hours in the future.

We will visit children at home as well as in the clinic setting as appropriate. We will visit schools/nurseries if required.

How to make a referral

Anyone can refer a child to the Children's Community Nursing Team if the family has agreed. We welcome referrals from parents/carers, and children themselves.  Referrals can be made either by:
