Multi-agency referral form for allegations against a professional

Workers in other agencies should use this form to make a referral to Social Services of a professional / volunteer against whom an allegation has been made in line with Chapter 15 of the London Child Protection Procedures, or Chapter 5 of Safeguarding Children and Safer Recruitment in Education.
