This service will provide you help with personal care and managing practical tasks in your own home. The service will also help you by providing advice and assistance with welfare benefit issues, assist you to develop daily living skills or enable you to transfer skills learnt in other environments e.g. Day Centres to your own living environment.
Who can have this service?
- you should live in Waltham Forest
- you must be aged 18 or over
- you are experiencing a level of mental ill health that significantly effects your ability to cope in the community
Will I have to pay towards this service?
We do charge for this service. We will tell you how much this is likely to be as part of your Community Care Assessment. See Community Care Charges for details.
Who provides this service?
There are Social Services Mental Health Domiciliary Workers based at a Centre in Walthamstow. They will work with you in your own home and in the community.
How to get this service
Contact your Locality Mental Health Team. There are two Locality Mental Health Teams for people aged 18 to 64. There is also a Team for People aged 65 and over (this is based in two offices).
Other information
- A written statement of the type of services you will receive, including the days and appropriate times, will be provided.
- You will be notified of all changes in service arrangements
- Your allocated times will be reviewed and negotiated with those involved who will be affected by the changes
- You will be given a named Placement Manager and a telephone number to call for queries and emergencies
