Community Learning Disability Nursing Team: what do we provide?
We provide assessment, advice, supported access and education regarding an individual's health need (which can be identified under physical, mental or psychological health)
The community nursing team also provides the following -
- one-to-one case-load working
- health promotion clinics
- epilepsy awareness/ rectal diazepam training
- healthy eating group
- body care awareness group
- sexual health clinic
- lead on Valuing People health focused projects
- nurses relating to sexual health and epilepsy
- links and training to non learning disability trained and experienced staff within acute primary and mental health care
The role of the community nurse is to facilitate access to health treatment and intervention through training and development, enabling the client, family or staff to develop the skills to meet the health need in the future where possible.
Who can have this service?
Adults with a learning disability living in Waltham Forest who are assessed as needing the service.
Will I have to pay?
No. It is provided free, according to health need.
The community nursing team operates between 9am –5pm Monday to Friday.
