All licences are issued subject to compliance with conditions and completion of the appropriate application forms in addition to payment of the licence fee.
The application forms are provided as WORD or PDF files. Just click on a form to download it.
You can print the form out and fill it in by hand, or fill in the details first using a computer and then print it out. However, all these forms must be signed by hand for the Council to consider your application. When you have finished, check you have answered all the questions and signed the form, then send it back to us. The correct address is shown on each form.
